Getting Started:
Review the 2012 Athletic Field Scheduling Guide – The Guide includes everything about the field scheduling and allocation processes, including billing due dates and field care responsibilities. Quick Links to Sections of the Athletic Field Scheduling Guide:

Requesting Fields:
For your convenience, forms can be submitted via email. If a signature is required, request a Client ID# to send forms via email or submit via scanned email, fax, mail, or drop-off with a signature.
- Acknowledgement of Receipt – Proof that you received a copy and have read and understood the Athletic Field Scheduling Guide.
- Field Use Request Form – Official form to provide the Parks Scheduling Office with the Organizations’ contact information and details about the field time being requested.
- Field Use Request Form – Overflow Page (if needed) – Page to submit with the Field Use Request Forms if additional lines are needed for the field time being requested.
- Athletic Field Use Organization Information - Information regarding the groups requesting field time.
- Certificate of Insurance – Naming the City of Bellevue as an additional insured for $1 million General Liability Coverage
- List of Board Members (if applicable) – Including names, addresses, phone numbers, and email addresses
- Proof of Non-Profit Status (if applicable) – Certificate or letter from IRS
- Credit Card Authorization Form (if applicable) – to authorize the City of Bellevue to keep the account number on file for future billings.
Before and During the Season:
- Concussion Compliance Form - To confirm your organization's compliance to the Zackery Lystedt Law (HB-1824 - Head Injury).
- Return to Inventory Form – To cancel unneeded reservations to save money and help meet needs of additional groups. Credit is issued for field time returned with at least 5 working days’ notice.
- Game Schedule Form – Once your game schedule is set, please submit it to the Parks Scheduling Office to ensure that fields are maintained and ready for your use. Note: fields maintained by the City field crew will be dragged. Some organizations are responsible for field lining. Two weeks’ notice is requested to get the reservations updated for the crew in a timely manner.
- Credit Request Form – Unable to use a reserved field due to field conditions (rainout, lights, etc…)? Groups may be eligible for credit. Preferred within five working days; not considered after a month.
- Field Payments – Payment of monthly field fees are due to the Parks Scheduling Office by the 15th of the month prior.
After the Season:
Within one month after the season has finished, please submit these forms:
