Confidence Test Report Forms
The Bellevue Fire Prevention Division has recently developed and revised its confidence test reporting system. Effective July 1, several types of test reports must now be submitted on approved Fire Department forms. The Confidence Test Cover Sheet explains more and also indicates which test reports are to be submitted specifically on Bellevue forms.

A cover sheet form must be submitted with every confidence test report. The cover sheet is both an effort to reduce the amount of information that has to be provided for each system report submitted, as well as a means for the Fire Department to obtain the information it needs for each system.

For example, building owners only need to provide the pertinent information for their testing company on the cover sheet. That information does not have to be repeated on each individual sprinkler system test form.

ALL TEST REPORTS SHALL BE SUBMITTED WITH A COVER SHEET REGARDLESS OF THE NUMBER OF SYSTEMS YOU ARE TESTING. Any reports received without a cover sheet will be returned and noted “INCOMPLETE.” You must provide all information in all fields pertinent to your particular test(s).
